Same here, but I prefer to stay on neomutt-20180716, despite being a
contributor to the upstream project.

Sorry, I don't understand what "physically" means.  What command do you

That sounds a lot like neomutt is confused about creating a file (mbox)
versus a directory (maildir).  Again, I'll need to know what command you
use for syncing to be able to go into more detail.

FWIW, here is how I sync: I do not divide my mails into separate folders
on the server.  Everything is either suspected spam which I inspect once
in a long while manually, or it goes into my inbox maildir.  My sync
script just tars the ./new/ subdirectory, rsyncs the tar file over to my
desktop, unpacks it, and then feeds each email to procmail; procmail
takes care of splitting them into folders.  After all of the above is
successfully completed, the script moves the emails from ./new/ to
./cur/ on the server.

I omitted some details related to mailing lists and getting rid of the
hated "personal" copies :-P
