Just a shot in the dark, I would also test it with ncurses instead of
slang. For example, in the past I wasn’t able to get colourising to work 
with xfce4-terminal and rxvt-unicode neither within Mutt (with several 
NeoMutt patches applied) nor with NeoMutt itself. Some of my colour 
definitions worked (5 out of 40) but they always changed from session to 
session. After switching to ncurses all works as it should.

I you have the time and being interested in, have a look at [1].

I cannot say anything about OS X terminal, but I think when something 
doesn’t work, it may be worth not to put all and every previous setting 
in your new configuration. I would start with a small basic config and 
then check several colour settings (within NeoMutt’s command line).
