It seems I got mutt into a strange state with my various attempts.
Quitting and restarting mutt in a clean state (i.e., according to my
config) fixed it.

Extracting the password from an encrypted file is a far better idea
than putting it in your config.  As a first step in getting it
working, though, I chose to provide it manually.
